Dann wird das Arthroskop in den Schleimbeutel unter das Schulterdach (Bursa subacromialis) eingeschoben (hierfür ist kein neuerlicher Hautschnitt erforderlich) und der Schleimbeutel (falls verkalkt oder entzündet) mittels eines Saugmessers (4,5 mm Durchmesser) entfernt.
Dazu benötigt man einen zweiten 5 bis 8 mm großen Hautschnitt.
